A peer-reviewed study published today in the high-impact journal Clinical Infectious Diseases (NIH-funded) has documented exceptionally high rates of viral RNA shedding following receipt of the live attenuated influenza vaccine (LAIV/FluMist).

In a group of 283 healthy adults aged 18–49, researchers detected influenza A or B RNA in nasal samples from 91.2% of participants within the first week post-vaccination.

Shedding peaked at up to 92% on Day 1, remained detectable in 23% by Day 5, and was still present in 9% on Day 7.

The FluMist vaccine contains live, laboratory-engineered chimeric (reassortant) influenza viruses—hybrid strains created by combining genetic material from a master donor virus with surface antigens from circulating wild-type strains.

These viruses are intentionally designed to replicate in the nasopharynx to trigger an immune response, which also enables them to be shed through nasal secretions.

According to the FDA’s official package insert, these vaccine viruses can be cultured as infectious material and may be shed for up to 28 days after vaccination.

Clinical trials have documented positive cultures of live vaccine virus as late as Days 23, 25, and 28 in some recipients.

Critically, the insert also confirms a documented case of vaccine-derived Type B influenza transmission: an unvaccinated placebo recipient in a daycare setting developed mild symptomatic infection after exposure to a vaccinated child.

While the new study measured RNA detection (which does not always equate to infectious virus), its findings align with FDA data on culturable, replication-competent vaccine virus.

The study authors note that any potential for secondary transmission would be greatest in the immediate days following vaccination.

This raises substantial public health and informed consent concerns.

Recipients of the nasal flu vaccine can temporarily become carriers capable of spreading vaccine-strain influenza virus to close contacts—including infants, elderly family members, immunocompromised individuals, and coworkers—often without awareness or adequate warning.

Many patients and their households are not routinely informed of these shedding and transmission dynamics when choosing or recommending the nasal spray formulation.

Greater transparency from healthcare providers and public health authorities is essential to ensure informed decision-making and protect vulnerable populations.
