Three trials, three different kinds of dental pain

One trial followed 270 patients recovering from wisdom tooth removal.  Researchers compared a eugenol-based paste with a standard antiseptic gel to prevent painful dry socket.

A second followed 100 patients undergoing pulpotomy, a procedure that removes inflamed nerve tissue from a tooth.  A third followed 92 patients with acute pulpitis, a deep and often severe toothache.

Across all three settings, eugenol performed consistently well.  Researchers overseeing the pulpotomy trial went so far as to recommend eugenol as the first choice for the procedure.  The pulpitis trial found that combining eugenol with standard treatment eased pain more effectively than standard treatment alone.

So why did dentistry pull back from something that works this well

The honest answer has little to do with pain relief.  The real story is modern bonding technique.

Eugenol interferes with the chemical reaction that hardens today’s resin-based fillings and adhesives, the materials most practices now build their entire workflow around.  That single compatibility problem, not any failure to relieve pain, pushed eugenol-based materials toward the margins of general practice.

Zinc oxide eugenol cement still shows up in temporary work and certain root canal procedures, but rarely anywhere resin bonding follows.  A material can outperform in comfort and antibacterial action and still lose ground for reasons unrelated to the patient in the chair.

The effect works similarly to that of numbing agents.  Eugenol also calms the inflammatory chemicals that build up around an irritated tooth, including several that are directly linked to swelling and throbbing pain.
