What Happens to Your Body When You Stop Drinking
People looking up this question are usually looking for a reason to keep going, or trying to work out what they are in for. Both are reasonable, and the honest answer has two halves that need to be taken together.

The recovery is real and more substantial than most people expect. The first few days can also be medically dangerous, and that part gets left out of most articles on the subject.

Alcohol withdrawal is one of the few withdrawal syndromes that can kill you. Opioid withdrawal is severe and rarely fatal. Alcohol withdrawal, in someone with significant physical dependence, can produce seizures and a condition called delirium tremens that carries a genuine mortality risk.
Do not stop abruptly on your own if any of the following apply:
- You drink daily or nearly daily, particularly in larger amounts
- You drink in the morning, or to settle shaking and nausea
- You have experienced tremor, sweating, or anxiety when you have gone without
- You have had a withdrawal seizure before
- You have been through severe withdrawal previously
- You have significant medical conditions
There is also a phenomenon worth knowing about: repeated cycles of heavy drinking and withdrawal tend to make each subsequent withdrawal worse rather than easier. Someone who has detoxed several times is at higher risk than someone doing it for the first time, not lower.
A medical evaluation before stopping is the appropriate step. Treatment for alcohol addiction should begin with that assessment rather than with a decision made alone on a Sunday night.
The first 72 hours
Six to twelve hours. Early symptoms appear in people with physical dependence: tremor, anxiety, sweating, nausea, headache, elevated heart rate, difficulty sleeping.
Twelve to twenty four hours. Symptoms intensify. Some people experience hallucinations while otherwise remaining oriented, which is frightening and is not the same as delirium tremens.
Twenty four to forty eight hours. The highest risk window for withdrawal seizures.
Forty eight to seventy two hours. The window in which delirium tremens can develop, involving confusion, disorientation, fever, severe agitation, and dangerous instability in heart rate and blood pressure. It is a medical emergency and requires hospital level care.

The first week

Months one through three

Immune function improves. Heavy drinking suppresses immune response, and people commonly notice fewer minor illnesses.
Blood pressure and cardiovascular markers improve.
Concentration and memory sharpen. Many people describe realizing how much mental fog they had been carrying only once it lifts.
Three months to a year
Cognitive recovery continues. Some changes associated with heavy drinking improve over extended periods of abstinence.
Long term health risk declines. Alcohol is associated with elevated risk of several cancers and of cardiovascular disease, and that risk decreases with sustained abstinence, though not instantly and not always to baseline.
Post acute symptoms fade. Some people experience months of intermittent mood swings, disrupted sleep, low energy, and periods of flatness. These come in waves and are frequently mistaken for evidence that recovery is not working. They settle.
The part that is not physical
The body recovers more predictably than the rest of it.
What alcohol was doing, socially and emotionally, does not disappear because the drinking has. If it was managing anxiety, the anxiety returns and now needs addressing directly. If it was filling evenings, the evenings are empty. If it was the mechanism for socializing, the social life requires rebuilding.
