Chicago White Sox pitching prospect faces another major elbow setback
At just 23 years old, Chicago White Sox pitching prospect Tanner McDougal, currently the organization’s No. 6 overall prospect and third-highest ranked pitcher, is dealing with his second significant elbow-related injury since being selected in the fifth round out of high school. Shortly after being drafted in 2021, McDougal tore his UCL, underwent Tommy John surgery, and missed the entire 2022 season.
Now, a few years later and after previously battling a hamstring injury, McDougal is once again experiencing issues with his throwing arm, this time diagnosed as forearm tightness. The injury forced him to exit his most recent Triple-A start after he was pulled while warming up for the second inning.

After not making his first full professional season debut until 2023, McDougal endured some early struggles across his first two seasons/43 minor league starts. In 2023, he posted a 4.15 ERA at Low-A, and followed that with a 6.04 ERA in 2024 across both Low-A and High-A levels. However, he began to turn things around last season, recording a 3.26 ERA over 28 starts and earning a promotion to Double-A.
This year, he had looked sharp in Triple-A, showcasing a high-velocity fastball alongside strong breaking pitches in both his slider and curveball. However, that progress now appears to be on hold as the Chicago White Sox will shut him down from throwing for a few weeks. Depending on how he responds, and if this is something that can be managed without surgery or if it is a more serious issue requiring a procedure, McDougal could be facing a significant amount of missed time this season, and potentially into next year depending on the severity.
