What Is a Root Canal—and Why the Debate?

Root canal treatment has long been the standard approach to saving infected or damaged teeth. The process involves removing the nerve and pulp, cleaning and disinfecting the interior, and sealing the tooth to prevent further infection.

Dentists have considered it a success when pain goes away and x-rays show improvement. But new technologies and old concerns are causing some in the dental world to ask whether this method truly eliminates infection… or simply buries it deeper.

Anatomy of a Root Canal


Research shows that many treated teeth still contain bacteria, fungi, and even yeast that can linger indefinitely and may contribute to other health problems.

A root canal is not a single canal but a complex system of tiny branches and microscopic tubules—about three miles of them per tooth. Standard instruments and techniques often fail to clean 30 to 40 percent of this system.

So when dentists say the tooth is sterilized, it’s more of a best effort than a guarantee. Research shows that many treated teeth still contain bacteria, fungi, and even yeast that can linger indefinitely and may contribute to other health problems.

Linking Root Canals to Chronic Illness

The 2018 documentary Root Cause highlighted a controversial claim: that a majority of women with breast cancer had a root canal-treated tooth on the same side as their tumor. A separate comment from a respected cardiologist claimed root canals aren’t just correlated with heart disease—they’re a cause. While these statements are debated, they reflect a growing suspicion that lingering infections in dead teeth may contribute to chronic systemic illness.

Peer-reviewed studies have confirmed that root-canaled teeth often contain bacteria like Enterococcus faecalis, Fusobacterium nucleatum, and others that can travel through the body, resist antibiotics, and evade immune defenses.

One study found that over half of treated teeth had bacteria deep in their tubules all the way to the enamel junction. In another, 4 out of 10 infected root canal teeth showed heavy fungal invasion. These aren’t fringe claims—they’re published in mainstream endodontic journals.

Why So Many Failures?

It turns out one of the biggest reasons root canals fail is operator error. General dentists, not trained endodontists, often perform the procedure without fully understanding the complex canal anatomy. Even skilled dentists may be using outdated tools or missing microscopic branches where biofilm—a sticky matrix of bacteria—hides. Biofilm is highly resistant to antibiotics and can survive traditional sterilization chemicals like sodium hypochlorite.

Unfortunately, even the chemicals themselves come with risks. Sodium hypochlorite can damage surrounding tissue, enter the bloodstream, and kill off healthy fibroblasts responsible for healing. This adds another layer of concern for patients already battling illness or inflammation.

Modern Alternatives to Traditional Root Canals

Despite all this, not every root canal needs to be condemned. New biological and technological approaches are emerging that offer more complete sterilization and less invasive solutions.

One promising area is called vital pulp therapy. Instead of removing the entire nerve, dentists use lasers and bioceramic materials to remove only the damaged portion. In some cases, the tooth’s stem cells regenerate new dentin, keeping the tooth alive and healthy. This kind of treatment has shown success using platelet-rich fibrin (PRF) and high-tech tools like the GentleWave or LightWalker lasers.

Ozone gas and ozonated water are also becoming popular tools in biologic dentistry. Studies dating back to 2004 show that ozone significantly reduces harmful bacteria without the toxicity of chemical irrigants. More recent studies show it can even inhibit yeast and other resistant organisms. Some dentists have experimented with injecting ozone directly into infected bone or soft tissue—often with impressive results.

In one case, a patient with bisphosphonate-induced bone death (commonly seen in cancer patients) underwent ozone injections that completely cleared the infection. The affected bone fragment eventually detached on its own, and follow-up scans showed the area had healed.

When Root Canals Lead to “Cavitations”

One of the least understood but increasingly acknowledged risks of failed root canals is the formation of cavitations—areas of dead bone left behind after a poorly cleaned extraction or failed endodontic procedure. Cavitations often don’t show up on standard x-rays and rarely cause pain, but they can harbor bacteria and toxins that affect the body systemically.

Diagnosing cavitations requires 3D cone beam imaging, which allows practitioners to see the full depth and density of the bone. Treatments vary from surgical debridement to non-invasive ozone injections, but the key is accurate diagnosis and individualized care.

Empowering Patients With Alternatives

Perhaps the most important takeaway is this: every patient is different. Some people may do just fine with a traditional root canal. Others may struggle with chronic fatigue, inflammation, or illness that seems to trace back to dental work. A growing number of dentists now believe that patients deserve all the information—risks, alternatives, and emerging technologies—so they can decide what’s best for their own health.
